Application process
Visit the website to download an application and return to City of Berwyn - Senior Services. If under the age of 60, proof of disability must be submitted. After the application is submitted and approved, contact (708) 484-2510 between 8:00am and 4:00pm, Monday through Friday to schedule a ride. Reservations must be made at least 7-10 days in advance.
