Maintains records and issues certificates of vital records (birth certificates, civil union certificates, marriage certificates, and death certificates). Also issues recreation licenses, liquor licenses, and video gaming licenses for unincorporated areas of Ogle County. Provides for the legal registration of votes and maintenance of the voting record.

Fee
- $22 for a birth certificate ($4 for each additional copy ordered at the same time). - $26 for a death certificate ($8 for each additional copy ordered at the same time). - $22 for a certified copy of a marriage license/civil union certificate ($4 for each additional copy ordered at the same time). - $40 for a marriage license/civil union certificate. - Processing fees are charged for online and fax requests. No refunds are issued for online requests. - Cash or check accepted for in-office requests.
Eligibility
Birth Certificates: Must be one of the following to obtain a certified copy of a birth certificate: - A legal guardian with certified court documentation proving verification of guardianship. - A legal representative acting on behalf of the person named on the birth certificate (must have authorized documentation). - An agent approved by the state registrar. - Adult 18+ (or an emancipated minor with certified court documentation) if individual is request their own birth certificate. - The mother or father (if listed on the certificate) of the child whose certificate is being requested. Death Certificates: Must be one of the following to obtain a certified copy of a death certificate: - Executor or administrator of the decedent's estate. - Someone who has a personal or property right interest in the certificate. - The informant listed on the death certificate. - The next of kin. Civil Union Licenses: - Adult age 18+. - Not be an ancestor and a descendent or between siblings whether the relationship is by the half or the whole blood, or by adoption; an aunt or uncle and a niece or nephew, whether the relationship by the half or the whole blood, or by adoption; or between first cousins. - Not enter into civil union prior to dissolution of a marriage or civil union or substantially similar legal relationship. - Individuals who reside out of the state may not apply for a Civil Union license in Illinois if such Civil Union would not be legal in their home state. Copies of Marriage License/Civil Union Certificates: - Anyone outside of the couple must have a valid reason for obtaining the certified copy of the license/certificate on the couples behalf.
