Employer names and dates for the past two years, d. Self-employment income and type of business, e. Bank information to set up your direct deposit, f. Information on family members who may be eligible to receive benefits on your record (2) Call National Toll-Free number to schedule an in-person appointment or visit website to begin the application process or for more information at www.ssa.gov/benefits/retirement/ (3) Bring required documents
