Contact your local Care Coordination Units (CCUs) to request a screening. You can call the Senior HelpLine to locate your Care Coordination Unit (CCU).

Care Coordinators from Care Coordination Units (CCUs) provide a free Choices for Care screening consultation to help you, your family, and/or any authorized representative learn about your care options. Screens are primarily completed in hospitals before patient discharge, but they can also be completed in the community, at a Supportive Living Program location, or at a Nursing Facility.
