Services provided: - Family planning counseling and referral. - Pregnancy testing. - Prenatal counseling and follow-up. - Referral for medical card. - High risk infant follow-up. - Family case management. - Lead poisoning testing and counseling. - WIC (Women, Infants, and Children Program). Pediatric Developmental Screening: Provides evaluation and assessment to determine whether there are developmental delays or other issues that need to be addressed. WIC: Provides healthy food, nutrition education and counseling, and breastfeeding support for pregnant and breastfeeding individuals, infants, and children. Also provides referrals for health care and other services. Family Case Management: Provides family case management services to pregnant individuals, infants, and children with high-risk medical conditions. Offers assistance with: - Finding a doctor for prenatal care. - Finding a pediatric doctor. - Understanding proper nutrition. - Understanding the stages of child development. - Getting information on how to become a better parent. - Understanding the importance of prenatal care. - Learning the signs and symptoms of 0-term labor. - Understanding the importance of regularly scheduled well child visits. - Understanding the importance of immunizations. Family Case Management can also help connect individuals with other resources, including: - WIC. - Family Planning. - Health Education. - Hearing/vision. - Substance abuse. - Prenatal/parenting classes. - Child care. - Housing. - Smoking cessation. - Lead screening. - Transportation. - Early intervention. - Immunizations. - Well child exams. - All Kids (health insurance). - Domestic violence.
