Application process

Visit the website to download and complete the Client Agreement Form. Completed forms should be mailed, faxed, or delivered in person to Proviso Township Hall. Once registered, call (708) 344-7430 to schedule a ride. Rides must be scheduled at least two days in advance.

Description

Proviso Township offers free rides for residents at least 60 years of age who need transportation to and from medical facilities and other qualifying non-medical locations within the Township. All participants must reside in Proviso Township. The Senior Ride Program is a curb-to-curb service that requires all participants to be able to walk unassisted to and from the Township vehicle.
