Habilitative Systems, Inc. provides community-based outreach, mental health, substance abuse, case management, and housing services. Services include screening, assessment, linkage, and referrals to appropriate levels of care for homeless adults. The program's goal is to reduce homelessness, and recidivism and stabilize support systems.
