Eligibility

Age 18 years and under age 60 - Individuals age 60 years and older with a diagnosis of Traumatic Brain Injury (TBI) or HIV/AIDS - Illinois resident - US Citizen - Living in an institutionalized setting for a minimum of 90 days - Must be Medicaid eligible or receiving benefits - Physician approval for a safe transition
