Signed Cunningham Township Supervisor's Office consent to release information form (attached to the back of the application), government photo ID, proof of current address or of homelessness, bank statement(s), pay stubs if applicable, social security benefit letter if applicable, proof of public aids such as DHS, if able to work, will need a UI Finding from the unemployment office, if unable to work, will need a Doctor's Note Stating Inability to Work & Dates of Restrictions, letter from employer to verify medical leave (if applicable), if pregnant or with children under 18 in the household, will need Proof of An Application for Temporary Assistance for Needy Families (TANF)

Must email, text, mail, or come to the office to drop off a filled out application with all necessary documentation, applications can be found on the website and are available 24 hours a day outside of the front door to the office, call if assistance is needed for obtaining or filling out an application, for help with emailing, scanning, printing or texting the documentation, visit the Urbana Library across the street from the Township - individuals can receive free printing and scanning services from the library if they inform the library that they are needing help to fill out an application or submit documentation for Cunningham Township

Description

Provides temporary support, up to $425 per month, for Urbana residents who are out of work or disabled. Participants cannot be receiving any other cash benefit such as SSI/SSDI, TANF, or AABD. Assistance is provided regardless of immigration or criminal conviction status to the extent allowed by law.

Free printing, scanning and help with emailing documentation can be found at the Urbana Library, across the street from the Township.
