Call for an appointment or visit during program hours Documents required: - Photo ID (State issued ID, birth certificate). - Social Security cards of all household members. - Copy of rental, lease, or mortgage agreement. - Utility bills - Proof of income - Eviction or disconnection notice, if applicable. - Citizenship papers (if applicable) - Marriage/divorce/separation papers (if applicable) - Military service records (if applicable) - Prison record card (if applicable) - Illinois Department of Health Resource Records (Cash Assistance, Medical Assistance, Food Stamps) - Veterans Benefits and Workers Compensation papers (if applicable) - Unemployment Insurance - Retirement Benefits - Income tax returns - Most recent bank statements from all accounts - Titles/registrations for all vehicles
Eligibility
- Must reside in Warren Township for at least 6 months - Must exhibit and document financial hardship - Must be able to show ongoing ability to pay after assistance is granted
