PAF Co-Pay Relief Program

Patient Advocate Foundation (PAF) provides direct financial assistance to insured patients who meet certain qualifications to help them pay for the prescriptions and/or treatments they need. This assistance helps patients afford the out-of-pocket costs for these items that their insurance companies require.

Application process

Complete the online application for assistance, entering all of the required information. Once the individual has fully completed the application, they will be notified immediately if they are eligible for financial assistance. If the individual is eligible for assistance, the application will be instantly approved, and they will be enrolled in the program. They will have immediate access to their award and pharmacy card. Must be currently insured and have coverage for medication seeking financial assistanceHave a confirmed diagnosis and treatment planMust reside and receive treatment in the United StatesIncome must fall at or below 400% or 500% of the Federal Poverty Guideline (FPG) with consideration for the Cost of Living Index (COLI) and number in the household
