- Community Supports for Independence Program: Begins with an independent living skills assessment that allows the team to customize supports to increase skills in areas of need. Goal of the program is to support the participants with achieving their goal to live independently within two years. The program allows individuals with developmental disabilities the opportunity to gain skills and build their natural supports over the course of two years. - Transitional In-Home Program: Serves individuals who have acquired brain injury or head trauma after the age of 22 years. Participants are assigned to a service coordinator through the Department of Health. The service includes training and practice with activities related to daily living and maintenance of a household.
