Mental Health Drop In Centers | LAKE COUNTY HEALTH DEPARTMENT - Williams Consent Decree, New Hope Recovery Center

Provides transition coordination and support to individuals with mental illness. Prepares and helps them moved from state-funded facilities into permanent supportive housing.
