Eligibility

Must be ages 60 and older, a U.S. citizens or eligible non-citizens within the specific categories, resident of Illinois, have non-exempt assets of $17,500 or less (home, car, or personal furnishings are classified as exempt assets), and have an assessed need for long term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment)

Provides Care coordinators trained by the Illinois Department on Aging that can meet with individuals and their families in the home. This is a free consultation to provide counseling and education about available senior services and then arrange and link the older adult to programs that are needed.
