Grand Prairie Services (GPS) Specialized ACT services are provided to consumers at greatest risk of rehospitalization. These comprehensive team-based services are provided to adults with serious persistent mental illness. They are consistent with an evidence-based practice model. ACT provides consumers with access to a highly qualified, multidisciplinary team of practitioners who provide most services in the community. Individuals served by these teams often require outreach to remain engaged in services.

Grand Prairie Services (GPS) is a not-for-profit integrated healthcare provider with over 73 years of experience providing outpatient counseling, mental health services and primary care. The organization began in 1950 as a counseling agency in Park Forest, and then merged in 1956 with social service groups representing Park Forest, Chicago Heights, Homewood and Harvey.
