Photo ID (passport, state ID, driver's license or matricula consular); Proof of income for household (copies of last 6 weeks of paystubs, unemployment statement, Social Security awards letter or pension, proof of income of person(s) helping application financially and a support letter written by them); Proof of address (utility bill issued within last 60 days, driver's license or state ID issued within last 30 days, current lease or proof of current mortgage).

Eligibility

Must be a resident of DuPage County for at least the past 30 days; Must be between 19 - 64 years of age; Must meet income guidelines (household income at or below 200% of the federal poverty level); Unable to access medical coverage and not eligible for select private or public health coverage.
