Photo ID (driver's license, State ID, Consular Card, or passport) Proof of current address (current utility bill, lease, benefit letter from Social Security Administration, Medicare, Medicaid, US Immigration/Asylum documents, shelter letter or hotel bill) Information for any other people who live in the household (first and last name, birthday, gender, race, country of birth) Income information for everyone living in household.
