Supportive Living Communities - Eden Chicago

Eden Supportive Living offers Supportive Living Facilities (SLFs) as an alternative to nursing home care for low-income persons with physical disabilities (22 and older) under Medicaid. The program is designed to preserve privacy and autonomy while emphasizing health and wellness for persons otherwise needing nursing facility care. Eden focuses specifically on people living with disabilities such as Multiple Sclerosis, TBI, stroke, kidney failure, spinal cord injury, kidney failure, and more. The facilities can provide recuperative and preventative nursing care, 24-hour response/security, meals, socialization, and more.Please contact individual communities for more information about eligibility and availability.
