MEMORY CARE HOME SOLUTIONS
Memory Care Home Solutions exists to improve dignity and quality of life for people living with dementia and their care partners by transforming evidence-based interventions into effective home and community-based healthcare soluations. MCHS addresses community need by maximizing function for persons with dementia and equipping care partners with skills and strategies to support them. Their evidence-based Dementia Care Services increase quality of life, reduce stress, increase knowledge and ability to manage ongoing care challenges, provide access/connection to resources, and reduce adverse health events. Services include a comprehensive assessment of the person with dementia, care consultation for the entire care partner team, on-going dementia care navigation, family training sessions, in-home occupational therapy, follow-up conferences, and connections to community resources. MCHS is also committed to providing outreach and education regionally and nationwide. ALSO KNOWN AS MCHS MEMORY CARE
