INTERMEDIATE CARE FACILITY | DECATUR MANOR HEALTHCARE

Provides an intermediate care facility for persons with chronic mental illness. Treatment in a residential housing setting, including Schizo-Affective Disorder, Major Depression, Bipolar Disorder, Schizophrenia, as well as those who are dually diagnosed with a mental illness and substance abuse. Residents are given the opportunity to gain a variety of skills to assist in the management of symptoms, improve social skills, increase independence, vocational training, and more.

- Evaluation and consultations by psychiatrists and medical physicians.

- 24-hour nursing care.

- Individualized case management.

- Psychiatric rehabilitation.

- Psycho-social rehabilitation.

- Medication management.

- Nutrition and dietary management.

- Individual and group therapy.

- Family therapy.

- Marital/couples therapy.

- Discharge planning.

- Therapeutic in-house work program, job training, and token economy.

- Transportation.

- Activities (including daily YWCA visits, local zoo outings, karaoke, Wii, and more).
