Support Services

Westside Health Authority works in collaboration with external agencies to provide for all of the employment, supportive services, and other needs of the formerly incarcerated. Services include case management, personal identification assistance, referrals for basic needs, public benefits assistance, and more.

Application process

Intake procedure includes: Orientation, TABE testing, clients will be assigned to case manager for individual assessment plans to meet and to provide for clients' supportive service needs; assigned to an employment specialist for job readiness training, individual career planning and job placement. Registration is held Monday through Friday at either 9:00am or 11:30am at 5417 West Division, Chicago IL 60651. Call for additional information
