Helps senior citizens, who might otherwise need nursing home care, to remain in their own homes by providing in-home and community-based services. Services include: - Comprehensive care coordination. - Adult day services. - In-home services (cleaning, meals, laundry, shopping/running errands, and personal hygiene). - Emergency home response service. - Automated medication dispenser service.
