IMMUNIZATIONS | CHAMPAIGN-URBANA PUBLIC HEALTH DISTRICT
Provides routine immunization to uninsured adults. Routine immunizations are also provided to child who might not otherwise be vaccinated because of inability to pay. Immunization offered include: - Diphtheria/Tetanus/Pertussis (DTaP). - Hepatitis B. - Haemophilus influenza type B (Hib). - Measles/Mumps/Rubella (MMR). - Meningococcal conjugate vaccine (type of vaccine used to protect adolescents from certain types of meningitis). - Pneumococcal conjugate vaccine (type of vaccine used to protect infants and toddlers from life-threatening meningitis and blood infections from certain bacteria). - Polio. - Tetanus/Diphtheria (Td). - Varicella (chickenpox). - Flue vaccine (offered seasonally). Preservative-free vaccine available by request for eligible children ages 6-35 months of age.

Eligibility
- Uninsured or underinsured adults. - Children through the age of 18 if they meet one or more of the following criteria: are Medicaid eligible, uninsured, are an American Indian or Alaska Native (AI/AN).
