Must be 18 years or older and a resident of Illinois. Must have active Medicaid. Must meet 3 of the below criteria and be diagnosed with Bipolar Disorder or a psychotic disorder (Schizophrenia, Schizoaffective, etc.): 1. Multiple and frequent psychiatric Inpatient admissions 2. Excessive use of crisis or emergency services with failed linkages 3. Chronic homelessness 4. Repeat arrests and incarcerations 5. Multiple service needs requiring intensive assertive efforts to ensure coordination among systems, services and providers 6. Functional deficits in maintaining treatment continuity, self-management of prescription medications, or independent community living skills 7. Persistent/severe psychiatric symptoms, serious behavioral difficulties, a co-occurring disorder, and/or a high relapse rate

Description

Provides intensive behavioral health treatment to adults with severe mental illness. Offers services in a community-based setting by an team of mental health professionals. Programs range from one of three levels of treatment intensity: - The Assertive Community Treatment - The less intensive Community Support - The Community Support Individual. Considered the final level of service before full self-sufficiency. Offers a multidisciplinary approach of intensive services provided where and when needed, 24 hours a day, seven days a week. Teams include peer support specialists and practitioners with expertise in: - Psychiatry - Nursing - Social work - Substance use treatment - Employment These teams initiate: - A housing search - Developing a care plan - Continuing to work with clients over the long-term.
