Case Management - Franciscan Outreach - East Garfield Park (Garfield Community Service Center)

Franciscan Outreach case managers conduct initial assessments and provide guidance to individuals experiencing homelessness. Case managers also provide individuals referrals to the following services: ID documents, Social Security benefits, public aid resources, veterans benefits, primary and mental health services, substance use disorder (SUD) treatment, legal aid, job training programs, employment opportunities, immigration services, and housing programs.Please note that case management is offered at the Pilsen location only for current guests of that shelter.

Guests at Franciscan Outreach have access to further services that help with stabilization, including support groups, public transportation assistance, monetary support for essential expenses, and programs to transition participants into permanent housing and/or employment.Franciscan Outreach also offers referrals for medical assistance, mental health counseling, healthcare, substance use disorder (SUD) counseling, job training, and legal aid.
