Linkage Care Management - Grand Prairie Services - ACCESS Department

Grand Prairie Services (GPS) Linkage Care Management services provide supportive services for individuals in all State-operated mental health facilities. Care management services include the provision of support such as planning, coordination, advocacy, and professional consultation with other providers of service and assistance with the transition of living arrangements.This program ensures that consumers who have been evaluated for or admitted to the State-operated psychiatric facility receive prompt follow-up care in the community. Linkage Care Management staff participate in discharge planning activities while the individual is in the hospital.
