COMMUNITY CARE PROGRAM (CCU) | CCSI CASE COORDINATION

Provides in-home assistance to seniors help them remain independent in the home. Services provide include: - Case Management. - Home Care Aid (non-medical in-home care to assist with activities of daily living/instrumental activities of daily living, such as bathing, grooming, dressing, housework, meal preparation, and transportation for essential errands). - Adult Day Service (currently not operating). - Emergency Home Response System. - Automated Medication Dispenser. - Money Management. Care Coordinators can also assist older adults and their caregivers in finding options when leaving the hospital or long-term care center.

Eligibility

Must meet all eligibility requirements: - 60 years old or older. - Either a U.S. citizen or eligible non-citizen within the specific categories. - Resident of Illinois. - Have non-exempt assets of $17,500 or less (home, car, or personal furnishings are classified as exempt assets). - Have an assessed need for long term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment).
