The admissions process begins with a phone call to the Social Worker at (773) 935-9600. She will send information about the home and ask for some preliminary information about current: financial situation; current living situation; current medical needs. Those who meet the low-income requirements and whose medical needs are within the ability to provide care, they will invite older adults and a family member to meet with them in person and tour the Home. This interview is their way of ensuring that they can provide a good fit in their care, and will also give a sense of what it would feel like to live at St. Mary's Home.
