You must submit a complete Intake Packet at least 3 days prior to your scheduled counseling session. You will be informed if any items are missing. Missing items must be turned in prior to your counseling session or your appointment will need to be rescheduled. Documents required: - Most recent mortgage statement from your lender, for all mortgage loans. - Any correspondence from your lender or attorney regarding the default - Foreclosure Court Summons (only if applicable) - Most recent 2 years of federal tax returns including W-2!s (If self-employed: Include last 2 years of business federal returns). - Last 60 days pay stubs for all household members age 18 years or older - Last 3 months of official bank statements for all accounts (all pages) (For business income: Include 3 months statements as well). Transaction histories will not be accepted. - ALL public assistance award letters, such as SNAP, WIC, SSI, SSDI, etc. - Most recent utility bills: telephone, cable/internet, gas, electric, water/sewer, trash - Most recent credit card statement(s) and all loan statements (i.e., car, installment, pay day loans, etc.) - Bankruptcy Discharge Documents (only if applicable) - Quit Claim Deed (only if applicable) - Copy of Divorce Decree (only if alimony or Child Support is received) - Homeowners Insurance Declaration Page - Mortgage Assistance Application form
