Visit during program hours and bring all required documents below Documents Required:

• Photo ID
• Recent utility bill as proof of residency
• Proof is required for the Reason for Assistance: SSI, TANF, Unemployment, LIHEAP, Senior Freeze, Medicaid, and/or Subsidized Housing
• Copy of the most recent Public Aid Medical card
• Social Security cards for all children in household
• Any others living in the home will need to bring in proof of their residency, such as mail received by them at the identified address

**Drivers Licenses are NOT accepted as identification for proof of residency; must bring recent utility bill
