Appointment Required. COVID-19 Notice: This location is not accepting new clients at this time. 1. Provide current TB results (no more than 30 days old). 2. The Client must bring a 30 day supply of medication. 3. Referrals after business hours must be made through the Chicago Department of Human Services. 4. At least one form of ID is required.
