Transitional housing available to individuals experiencing homelessness and HIV. Provides up to 24 months of housing with linkage and referrals to services, depending on individual needs. May provide the following referrals: - Medical services. - Mental health services. - Substance abuse treatment. - And more. If HIV-negative, see PC3 program.
