Eligibility
Must be residents of Illinois age 18 or older who are: - Cancer patients (including individuals who are newly diagnosed, in treatment or living with chronic or recurrent cancer) - Cancer survivors (ranging from individuals who recently completed treatment to survivors who completed treatment within the last five years) - Individuals with a loved one living with cancer who meets the above criteria - Individuals who experienced the death of a loved one due to cancer within the last five years
