Eligibility Requirements: Individuals or families must receive benefits under one of the following programs: -Supplemental Nutrition Assistance Program (SNAP) -Medicaid -Supplemental Security Income (SSI) -Federal Public Housing Assistance (FPHA) -Veterans Pension or Survivors Pension -Tribal Programs (and live on federally-recognized Tribal lands) (An individual can sign up if their child or dependent participates in any of the programs listed above.) -Consumers may also qualify for lifeline service if their household income is at or below 135% of federal Poverty Guidelines Fill out an application here, and then call the provider: https://nationalverifier.servicenowservices.com/lifeline
