Provides information and referral services to seniors and their caregivers. Offers an assessment of needs and connects them to local resources, such as: - Home delivered meals or community dining programs - Benefits programs to help pay for prescription drugs, health care, rent, utilities, and other needs - Family Caregiver Support for those who are giving unpaid care to adults with Alzheimer’s disease or a related disorder. - Transportation services (doctor appointments or personal errands) - In-home assistance ( light housework, personal care, and meal preparation) - Community engagement activities Also provide advocacy and follow-up to see that the services were received.
