Oak Street Health

Oak Street Health provides primary and specialty healthcare for older adults on Medicare. Oak Street also provides patient relations managers to help patients understand their benefits, coverage, and required documentation under Medicare. Plans accepted include Medicare Part A and Part B, MediGap, Medicare-Medicaid, and certain Medicare Advantage plans. Referrals for other specialists are also available to current patients.
