To request an appointment, email [email protected], call the office at (773)433-1995, or fill out the form on our website. Staff will assist in determining whether the LoSAH Center provides services that meet your needs and can answer any additional questions.

Commercial Insurance (BCBS PPO, CIGNA PPO, Illinois Medicare Part B_; Medicaid Managed Care (Illinois Medicaid - Medical Card, Aetna Better Health MCO, Illinicare/Cenpatico MCO, BCBS Community Options MCO, CountyCare MCO, Meridian MCO (WellCare), Molina MCO); The LoSAH Center of Hope is committed to ensuring that no individuals who meet the eligibility criteria are turned away due to inability to pay. Staff will assist during the intake process to identify appropriate options that align with the client's needs
Eligibility
Must live within the boundaries of the Logan Square, Avondale, and Hermosa Expanded Mental Health Services Program Area. The LoSAH Center will not base program eligibility on a person's insurance status or ability to pay and will not discriminate on the basis of an individual's race, color, sex, national origin, immigration status, disability, religion, age, sexual orientation, or gender identity.
