Assists individuals with disabilities to move out of nursing homes and live independently in their own home. Assists with identifying appropriate housing, find appropriate personal assistant and nursing care, supply some of the necessary household items, assist with deposits, rent, and utility hookups, and provide home modifications if necessary.
