Call to schedule an appointment Documents Required: - Birth Certificates/Proof of Citizenship - Lease, most recent rent receipt, or mortgage statement - Social Security Cards for all members in the household - Photo ID - Proof of Income - Employer letter verifying employment - Documents on benefits client is receiving - Bank statements - Proof of marriage, divorce documents, or proof of separation - Copy of most recent tax return - Letter of acceptance or denial from other agencies - Proof of emergency - Disconnection or eviction notice - Written verification that client is receiving or have applied for food stamps, TANF, medical card, unemployment benefits, SS/SSD/SSI, and child support
