VETERANS COMMUNITY ACTION TEAM (VCAT) | VETERANS PATH TO HOPE
Provides to veterans who have moved back into housing after being homeless. A team of veterans deliver furniture, food, and household items to help the veteran family transition into housing security.

Application process
Open to veterans are referred to VCAT through an agency case manager, a VA HUD-VASH case manager, or a case manager from a collateral agency that has a Memorandum of Understanding with Veterans Path to Hope.
