Application process

Appointment Required;-- All services are restricted to members of Local 1, 25, 45, 46, 47, and 48 and their family members. Also, individuals enrolled in Medicare can receive services. Though all services are covered through the employer's health plan, a copay might be required. Union Health Service (UHS) has a Medicare program that lets members use their traditional Medicare coverage with no out-of-pocket costs for services provided at a UHS facility.
