A care coordinator will complete an assessment to determine your eligibility for services. - Age 60 or older. - Determined physically in need of services and supports based on the comprehensive assessment tool. - Resident of Illinois. - U.S. Citizen or an eligible non-citizen within the specific categories. - Agree to apply, and if found eligible, enroll for Medicaid benefits on an annual basis.
