Application process

Must be referred by school district. Concerned parties may contact the student's school district to request referral. Documents Required: - Schools and/or districts must complete a formal referral on website - Copy of student's current attendance record - Copy of documentation showing the school/district attempted different types of interventions (home visits, parent meeting, school district letter, etc.) - Proof of sending a certified letter to the family with return receipt after the 10th unexcused absence
