Must meet income guidelines and reside within township. Must also be ineligible for any other government financial assistance. For emergency assistance must also be able to demonstrate immediate need, have utility shut-off or eviction notice and meet other non-financial qualifications.
General Assistance: - Completed General Assistance application - Photo ID (for applicant) - Social Security cards (for the applicant and everyone in the household) - Birth certificate (for everyone in the household) - Current lease or mortgage statement - All utility bills (must be in applicant's name) - Proof of all household income for the last 30 days, both earned and unearned - Recent bank statement, within the last 30 days - Proof of any IDHS benefits, such as SNAP or Medicaid (if receiving) - Verification of having applied for or have a pending case with Social Security (if applicable) - Proof of no income from IDES or IDHS (for anyone in the household over the age of 18 and not in high school with no income) Emergency Assistance: - Completed General Assistance application - Photo ID (for the applicant - Social Security cards (for the applicant and everyone in the household) - Current lease or mortgage statement - Proof of all household income for the last 30 days
