Eligibility

Birth certificates may only be released to a parent listed on the birth certificate or a legal guardian who is able to present court documentation as proof of their guardian status; Death certificates may only be released to relatives of the deceased or a non-relative who can prove a financial or property interest in the deceased, such as a beneficiary of a life insurance policy

Required documents

Must provide a photocopy of a photo ID of a parent named on the Birth Certificate, or proof of legal guardianship. You must bring a photo ID (preferably a driver's license) or documents verifying that you have a legal need for the death certificate (for example, if you are the executor of the estate of the deceased).

Call for general questions; Applications for records are available online, in person, and by mail; Written requests will be processed within 1 business day after being received

$21.00 for a birth certificate ($11.00 for each additional copy when purchased at the same time); $21.00 for a death certificate ($12.00 for each additional copy when purchased at the same time); Additional services fees apply if using a debit/credit card as payment
