Psychological, Physical, Functional assessment (I-CAP or SIBR), Photo ID, Social Security card, Guardianship papers if applicable, Individual Support Plan, if available, and/or Enrollment packet provided by referral source.

Call for information and intake, intake findings indicate appropriate for services (disability before age 20; intellectual disability/epilepsy/autism/cerebral palsy)

Description

Provides Community Day service for adults with developmentally disabilities. Offering a regular work program focusing on developing work habits. Subcontract jobs provide work experience and a paycheck for those individuals able to maintain working 5-1/2 hours at VOP or at supported employment location in the community.

Developmental program is comprised of paid work experiences, recreational, social, educational, and living skill opportunities.
