FAMILY CASE MANAGEMENT | WINNEBAGO COUNTY HEALTH DEPARTMENT

Provides support, education, and referrals to expecting families and families with infants. Also includes the Better Birth Outcomes (BBO) and Adverse Pregnancy Outcome Reporting System (APORS) programs. Adverse Pregnancy Outcome Reporting System (APORS): - Provides free home-visiting support for families with new babies with identified risk factors. - Public health nurses provide education, developmental screenings, and referrals. Better Birth Outcomes (BBO): - Women are enrolled in the program through pregnancy up to six weeks postpartum. Expectant mothers can get personalized support from WCHD during each stage of pregnancy receiving: - One-on-one support. - Coordination. - Education. - Guidance. - Access. One-on-One Support: - Monthly visits with a registered nurse to assure mother and baby are healthy. Coordination: - Coordination of care with doctors and other social service agencies. Education: - Prenantal health. - Nutrition. - Best safety practices while pregnant. Guidance: - How to properly wear a seatbelt while pregnant. - Identify and correct possible safety hazards in the home. - Implement and maintain safe sleep practices. Access: - Referrals to childbirth and parenting classes. - Referrals for obtaining baby supplies. - Referrals for food assistance programs. - Referrals to contraceptive services.
