House of Welcome Adult Day Services provides services for people with early memory loss, including mild cognitive impairment and early Alzheimer's disease or other dementia. There are various activities related to the day's theme, including cognitive exercises, creative arts therapies, discussion/reminiscence, and more.

Complete the Demographics & Health History form. Fill out the Personal History application. Download the HOW Medical Report and ask your family member's primary care doctor to fax it to HOW at (847) 242-6275. Forms may be emailed, mailed or dropped off at HOW, 1779 Winnetka Road, Northfield, IL, 60093, during business hours.
