Call or visit during program hours to obtain an application packet. After submitting application, a case worker will call back and schedule an intake appointment. Documents required: - Photo ID (Driver's license) - Proof of income - Proof of address - Social Security Card - Birth Certificate
Eligibility
Must be Wauconda Township residents 18 years of age or older and have previously applied to state and federal assistance programs. Also must meet income guidelines according to Township regulations.
