SENIOR INFORMATION SERVICE (SIS) OUTREACH & REFERRAL | VOLUNTEER SERVICES OF IROQUOIS COUNTY
Offers Senior Information Service (SIS) Outreach & Referral. Assistance is provided in our office, over the phone, during home visits by our Outreach SIS workers at our workshops held throughout Iroquois County. Assistance includes: - Answer all questions including qualification questions. - Assess clients' needs and make client referrals. - Help clients fill out and file application/documents required for various programs such as Benefit Access Application, P-Tax form, Senior Citizen Homestead Tax Freeze form, Medicare Part D and pharmaceutical programs, Medicaid/Snap applications, Social Security Extra Help program, and AARP Income Tax (only low income, times and days will vary each year). - Advocate on behalf of clients to assure access to needed services.
